Application for Services
Individuals can self-refer, or be referred by their doctor, family
members, friends, counselors, nursing home administrators, or hospital
discharge representatives.
To refer yourself or a loved one to
Hospice of Southwest Oklahoma, please
contact our office. We are currently working on an online
application that you will be able to submit via email. Please keep
checking back for updates.
** HSWO is a nonprofit, United Way organization
that promotes comprehensive hospice care services for the patient
and their family. We continuously provide community and professional
education to patients, professionals, and the families they
serve in issues relating to care giving, terminal illness, spiritual
comfort, loss and bereavement.
