Memorial Donations

Memorial donations are donations made after a loved one has passed away.  These types of donations are made by family members, friends, fellow church members, etc.  When memorial donations are received, HSWO sends the family a card letting them know that you have made a donation to our organization in memory of their loved one.  The amount is never disclosed.

Memorial donations can be made for anyone who has passed on, not just Hospice of Southwest Oklahoma patients.  If making a memorial donation for a loved one who is not a patient of HSWO, please make sure to include who you would like notified that the donation was made.

Memorial donations may be sent by mail, brought into our office or made over the phone (credit card only).  For more information, or to give a memorial donation, please contact our office at:
