Hospice of Southwest Oklahoma's Team

Our Hospice Home Care Professionals:

Hospice uses an "Interdisciplinary Team approach" to provide "holistic" health care.  Each team has a coach, a captain and several players.  This allows the patient and their family to have an active role in the care they need, want and will/will not accept with the guidance of their own trusted physician.  The coach is the patient and their family, the captain is the patient's personal physician, and the players are the nurses, social workers, bereavement counselors, home health aides, clergy, therapists and volunteers who provide their expertise in each specific component of care.  Our Director of Patient Care assigns a dedicated Registered Nurse, Licensed Practical Nurse and a Home Health Aide to each team.

Hospice of Southwest Oklahoma's interdisciplinary team also consists of three Associate Medical Directors.  If a patient needs medical attention and their doctor is unavailable, a physician is there to care for their needs. 

Each patient and their family is informed about all services which may be provided since they are not only part of the team but also recipients of the services.  Our health care portion of the team provides comprehensive palliative care aimed at relieving symptoms and pain, while the chaplains, social workers, grief counselors and volunteers provide the social, emotional, and spiritual support needed.
