Social Work Services

HSWO Social Work Services is dedicated to helping our patients and their families maintain independent living in their homes and provide vital information on different state and local programs that can assist the needs of each patient.  Several of these programs provide funding for the caregiver to help the family with hiring someone to help part-time with patient care.  Other programs apply to veterans and patients in nursing home facilities.  Some assistance is available to provide food or other needs depending on the financial situation of the home in which the patient is residing.

 

Help is also provided if needed with living wills, power of attorney, durable power of attorney, and Do Not Resuscitate (DNR) orders.  In addition, help is offered with real estate and trust issues and funeral arrangements if needed.

Support and counseling for depression and grief issues for any member of the family is available.  Often just being able to share feelings and emotions with someone who cares and understands can help get individuals through a very difficult time.  You are not alone with Hospice of Southwest Oklahoma.
